Reduce the number of Delayed Transfers of Care (DTOC) from hospital for residents of CWaC (total number of days delayed)


What does this mean?

A delayed transfer of care is when an adult patient in hospital is ready to go home, or move to a different setting but is prevented to do so.  This is sometimes called ‘bed blocking’ in the media. These delays can be caused by the patient being fit for discharge, but the appropriate services or support not being in-place at home, or wider housing issues.


Why is this important?

Delayed transfers of care are bad for people’s health and cost the NHS a lot – as they are keeping someone in hospital who could be discharged. Therefore, it is important that the Council and NHS work hard to make sure the right support is available to ensure people can be discharged.
