Reduce the number of Delayed Transfers of Care (DTOC) from hospital for residents of CWaC where the responsibility for the delay was social care only (total number of days delayed)
What does this mean?
A delayed transfer of care is when an adult patient in hospital is ready to go home or move to a different setting but is prevented to do so. These delays can be caused by the patient being fit for discharge but the appropriate services or support not being in-place. This measure shows how many delays were attributable to adult social care services.
Why is this important?
Delayed transfers of care are bad for people’s health and cost the NHS a lot – as they are keeping someone in hospital who could be discharged. Therefore, it is important that the Council and NHS work hard to make sure the right support is available to ensure people can be discharged.
